چکیده

bjectives The clinical importance of a leg length discrepancy (LLD) following total hip arthroplasty (THA) remains controversial. This tudy was undertaken to determine the effects of LLD on clinical outcomes at up to 3 years follow-up. esign and setting Prospective, multicentre study. articipants Nine hundred and eighty-seven cases of primary THA, categorised into two main groups: the NoLLD group (LLD of less than 0 mm) and the LLD group (LLD of 10 mm or more). ain outcome measures The primary outcome measure was the change in Oxford Hip Score ( OHS) at up to 3 years follow-up. Secondary utcome measures were length of operating time and hospital stay, and revision and dislocation rates. Potential predisposing factors for LLD, ncluding body mass index, age and type of anaesthesia employed, were examined. esults At 3 years, the LLD group had a significantly worse OHS [22.0; 95% confidence interval (CI) 20.5 to 23.5] compared with the oLLD group (23.8; 95% CI 23.1 to 24.5) (P= 0.034). There were no significant differences in revision (P= 0.389) or dislocation (P= 0.220) ates between the two groups. Use of an epidural was associated with a decreased incidence of developing an LLD of 10 mm (P= 0.004). onclusion A postoperative LLD of 10 mm or more leads to poorer functional outcomes. Further studies are needed to assess the impact of n LLD on clinical outcomes in the longer term. 2008 Chartered Society of Physiotherapy. Published by Elsevier Ltd. All rights reserved.

Leg Length Discrepancy in a Patient with Ipsilateral Total Knee and Total Hip Arthroplasty

Despite concerted intraoperative attempts to establish equal limb alignment, leg length discrepancy (LLD) remains a frequent complication following elective total hip arthroplasty (THA) [1]. A number of effective intraoperative imaging and clinical limb length techniques have been recommended to minimize this complication; however, all of these assessment modalities have their reported limitati...

An intraoperative device to restore femoral offset in total hip arthroplasty

BACKGROUND Leg length discrepancy (LLD) after total hip arthroplasty (THA) can lead to unsatisfactory outcome. Our objective was to design and evaluate a simple and reliable intraoperative device (Length-offset Lever) to minimize leg length discrepancy. METHODS This device was used in 51 patients undergoing primary total hip replacements. The leg length discrepancy was measured pre- and posto...

The impact of leg length discrepancy on clinical outcome of total hip arthroplasty: comparison of four measurement methods.

In a single-surgeon series of 119 patients with unilateral primary uncemented total hip arthroplasty, four leg-length discrepancy measurement methods (absolute, relative, trochanteric, standardized-trochanteric) were analyzed for their impact on WOMAC score, Oxford Hip Score and self-perceived leg-length discrepancy. After adjustment for age, gender and BMI, postoperative WOMAC scores correlate...

Hip resurfacing and conventional THA: comparison of acetabular bone stock removal, leg length, and offset.

The purpose of this study was to compare total hip arthroplasty (THA) and hip resurfacing arthroplasty (HRA) with regard to the amount of acetabular bone stock removed and the ability to restore leg length and offset. Anteroposterior pelvis radiographs of 153 consecutive THAs and 84 consecutive HRAs were compared. Excluded patients were those with prior hip surgery, those in which a best-fit ci...

Leg-length discrepancy after total hip arthroplasty.

Leg-length discrepancy after total hip arthroplasty can pose a substantial problem for the orthopaedic surgeon. Such discrepancy has been associated with complications including nerve palsy, low back pain, and abnormal gait. Careful preoperative measurement and assessment, as well as preoperative and postoperative patient education, are important factors in achieving an acceptable result. Howev...
